Community Breathlessness Programme

Transforming the clinical pathway for breathlessness with clinical decision
support software technology

We know the difference organisations can make in local communities. The right messages and access can inspire people to take control of their health, and that’s where our Community Breathlessness Programme comes in.

Designed to connect organisations with their communities, our programme enhances access to health services, helping re-engage people with the NHS and reduce health inequalities. From sports clubs to local groups, we tailor services to meet the needs of your population.

The Impact of undiagnosed breathlessness

Breathlessness affects more than 1 in 10 adults, often linked to respiratory or cardiovascular conditions, yet many remain undiagnosed or inadequately managed. Too often, it’s dismissed as ageing or low fitness, delaying treatment and early intervention.

The consequences extend far beyond physical discomfort:

  • Reduced activity & independence
  • Social isolation & anxiety
  • Repeated GP visits & hospital admissions
  • Increased healthcare costs and societal impact

By addressing breathlessness early, our Community Breathlessness Programme can improve quality of life, prevent avoidable admissions, and reduce long-term healthcare burden.

Commmunity-based respiratory programmes
LungHealth guided consultation
CardioLungHealth® pathway for undiagnosed breathlessness combines intelligent clinical decision support software with full clinical review

CardioLungHealth® intelligently incorporates evidence-based disease management guidelines to set a paradigm for the right diagnosis and management of patients.

The software enables clinicians to consistently review, accurately diagnose and tailor treatment to individual patient needs. Cardio LungHealth software will provide a thorough conclusive medical history so healthcare professionals can establish if chronic breathlessness is something new or has been recorded as a diagnosis or pre-existing condition.

Accurate COPD Diagnosis
CardioLungHealth® intelligent algorithms integrate findings from a structured clinical assessment
Asthma Identification
Intelligent interpretation of pre and post bronchodilator spirometry intra-consultation, FeNO and interpretation of changes in spirometry longitudinally over multiple consultations
Message Exchange for Social Care and Health (MESH)
CardioLungHealth has partnered with NHS England to enable direct data sharing between health and care organisations through a nationally recognised system
Early Detection and Intervention
CardioLungHealth® detects potential respiratory and cardiovascular issues when presenting with breathlessness
Improved Patient Safety
Medical alerts, developed with input from consultant respiratory physicians, nurse specialists, and patients, highlight clinical “red flags” and prompt further assessment when needed

Screening Service Flow

Arrival
Patient details are captured on the CardioLungHealth dashboard
Initial Assessment
• Point of care testing conducted for NT-ProBNP and Hba1C
• Blood pressure checked and 6-Lead ECG performed
Respiratory
• CardioLungHealth review conducted using CGU software
• Spirometry performed if determined at risk of COPD following results of CardioLungHealth review
• FeNo performed for suspected or know asthma
• Chest radiography referral for persistent cough more than 6 weeks
Cardiology
If NT-ProBNP ≥ 400 ng.l-1, subject undergoes AI echocardiography with images reviewed by the consultant cardiologist and heart failure treatment initiated as appropriate followed by referral for standard echocardiography as per the NICE heart failure pathway
Results
The CardioLungHealth Report/Letter, with Respiratory/Cardiovascular test results will be sent to the patients GP practice for action

BEAT Breathlessness: Transforming management of heart and lung disease in the Community

CardioLungHealth supported the award-winning BEAT Breathlessness project with Everton in the Community — England’s first community-based heart and lung screening hub.

Designed to support people living with chronic breathlessness, the initiative proved how local, proactive care can transform long-term condition management and improve outcomes. Its impact was recognised nationally, winning Most Impactful Partnership in Preventative Healthcare at the HSJ Partnership Awards.
